Becoming a Medicaid provider
- Must be 18 years old
- Hold a current birth doula certificate (see notes)
- Obtain your NPI Number
- Purchase liability insurance
- Submit provider application to MMAC
- Receive MMAC approval (may take several weeks)
- Apply to join Healthy Blue and Homestate Health within Availity (see notes)
- United Healthcare: Enroll in One Healthcare
- Apply to join United Healthcare within their Provider Platform
Notes
- Doula training must cover core competencies as stated in the Missouri Doula Guide.
- After applying to Healthy Blue, IMMEDIATELY go back in to see additional documents.
- Make sure to FULLY fill out every form Homestate Health requests when they contact you.

- The managed care companies may require you to be CPR certified.
- The managed care companies may require you to have HIPAA compliance training.
