Applying to Healthy Blue
- Complete all steps with MMAC and receive your acceptance email
- Create an account on Availity
- Find Healthy Blue under Payer Spaces
- Click on Provider Enrollment and Network Management
- Fill out the whole application
- Download, print, and fill out any requested documents
- Sign and upload documents
- Complete application and click “Submit”
- Monitor your application weekly in Availity until it says “complete.”
Notes
- Fill out ALL fields on every screen.
- Make sure all sections are printed and signed.
- After completing the first section of the application, it will likely ask for additional documents.
- There is a chance it may not give you a notification that additional documents are needed. You may have to go back into the application in order to continue.
- Watch your email, as they will email you a contract.
