  • Complete the application form
  • Carefully read and click below to agree to the Code of Ethics and Standards of Practice.
  • Submit a copy of your photo ID
  • Submit copies of all certificates necessary to complete application process
  • Include proof of HIPAA training
  • Submit membership fee

Notes

Medicaid
If you are joining the MDA and plan to accept Medicaid reimbursement, please make sure to review the information on what Missouri is requiring. We will give you feedback based on your application, to let you know any items you may need to complete to be eligible for Medicaid reimbursement.
